1)Please provide your CV and a brief overview of the type of research that you are interested in pursuing within the field of Resuscitation Science. Include which category of research are you interested in pursuing primarily (Clinical Research, Data Science/Epidemiology, Basic Science)? (1-2 page maximum)

2) Please provide a brief synopsis of your background and experience in research so far (1 page maximum)

3) Please provide a brief description of your overall career plans including your short- and long-term career goals and how 2-3 years of post-doctoral training under the T32 will help you achieve these goals? (1-2 page max.)
